El impacto del apego en la salud mental de población adoptada
Resumen
La adopción es más que el acto jurídico a través del cual una familia acoge de forma permanente a un menor que no puede permanecer con su familia de origen. La adopción es un proceso de adaptación del infante que comienza con una experiencia de ruptura temprana con su figura de apego y continúa con el establecimiento de un nuevo vínculo con las figuras adoptivas. Aunque numerosos estudios han registrado el impacto de la adopción en la salud mental, de la adopción en el apego y del apego en la salud mental, existe poca investigación sobre cómo interactúan las tres variables juntas. El objetivo de la presente revisión sistemática ha sido analizar el impacto del apego en la salud mental de población adoptada, considerando la historia previa de estos menores y su evolución a lo largo de los años. Se realizó una búsqueda en las bases de datos PubMed, PsycInfo y Psychology & Behavioral Sciences Collection. Se analizaron 7 estudios que atendían a los siguientes criterios de inclusión: a) artículos científicos, b) en inglés, c) cuantitativos descriptivos tanto ex post facto como longitudinales, d) realizados en población adoptada, e) que relacionaran las variables de apego, adopción y salud mental y f) con acceso al texto completo. Los resultados reflejan que las experiencias de privación temprana (y la duración de las mismas) guardarían relación con la gravedad de las conductas de apego. A su vez, estas conductas y un patrón de apego desorganizado (muy prevalente en población adoptada) correlacionarían con menor competencia emocional, más problemas internalizantes, peores habilidades verbales y un mayor deterioro cognitivo. Los autores hipotetizan que el apego mediaría entre la historia previa de adopción y algunos medidores de salud mental. Aunque se han registrado estos resultados, es necesario seguir investigando el papel del apego en la salud mental de población adoptada. Se proponen futuras líneas de investigación como estudios ex post facto que midan otras variables que puedan estar mediando entre la historia de adopción y la salud mental, y estudios longitudinales que permitan conocer cómo se modifica el apego a lo largo de los años y el impacto que eso tiene en los marcadores de salud mental de la población diana. Adoption is more than the legal act through which a family permanently takes in a minor who cannot remain with his or her family of origin. It is a process of adaptation of the infant that begins with an experience of early breakup with his or her attachment figure and continues with the establishment of a new bond with the adoptive figures. Although numerous studies have recorded the impact of adoption on mental health, the effect of adoption on attachment, and the impact of attachment on mental health, the interaction between these three variables has barely been investigated yet. The aim of the present systematic review was to analyze the impact of attachment on the mental health of the adopted population, considering these children´s prior history and their evolution over the years. A search was conducted in the databases PubMed, PsycInfo, and Psychology & Behavioral Sciences Collection. Seven studies were analyzed that met the following inclusion criteria: a) scientific articles, b) in English, c) quantitative descriptive both ex post facto and longitudinal, d) carried out in an adopted population, e) that related attachment, adoption and mental health and f) with access to the full text. The results show that early deprivation experiences (and their duration) are associated to the severity of attachment behaviors. In turn, these behaviors and a disorganized attachment pattern (very prevalent in the adopted population) would correlate with worse emotional competence, more internalizing problems, worse verbal skills and greater cognitive deterioration. Therefore, attachment would mediate between previous adoption history and some measures of mental health. Although these results have been recorded, it is necessary to continue investigating the role of attachment in the mental health of the adopted population. Future lines of research are proposed, such as ex post facto studies measuring other variables that may be mediating between the adoption history and mental health, and longitudinal studies that allow us to know how attachment changes over the years and the impact of that on the mental health markers of the target population.
