Contribuciones del Test de Rorschach al diagnóstico de la esquizofrenia en población adulta

Abstract

La esquizofrenia implica una desestructuración profunda de la identidad y del self, inaprensible mediante los cuestionarios de autoinforme tradicionales debido a la falta de autoobservación del paciente. El objetivo de este trabajo es analizar empíricamente las contribuciones del Test de Rorschach al diagnóstico de la esquizofrenia en población adulta, examinando sus indicadores estructurales bajo los sistemas SC y R-PAS. Los resultados indican que el test evalúa con gran precisión el colapso de la prueba de realidad y la escisión psíquica mediante indicadores como la calidad formal distorsionada (FQ-) y el Compuesto de Pensamiento y Percepción (TP-Comp). Asimismo, la prueba resulta fundamental para el diagnóstico diferencial frente al autismo o los trastornos disociativos y demuestra una sólida invulnerabilidad empírica frente a la simulación consciente. La evidencia disponible, sin embargo, advierte sobre ciertos retos metodológicos, especialmente el tamaño reducido de algunas muestras clínicas y la tensión existente entre el rigor estadístico cuantitativo y la riqueza interpretativa de la evaluación proyectiva. Se concluye que el Rorschach trasciende la simple suma matemática de marcadores diagnósticos, consolidándose como una herramienta clínica indispensable para comprender de manera integral el sufrimiento y el fragmentado mundo interno del paciente esquizofrénico.

Schizophrenia involves a profound destructuring of identity and the self, which is inaccessible through traditional self-report questionnaires due to the patient's lack of insight and self-observation. The objective of this study is to empirically analyze the contributions of the Rorschach Test to the diagnosis of schizophrenia in the adult population, examining its structural indicators under the CS (Comprehensive System) and R-PAS systems. The results indicate that the test evaluates with great precision the collapse of reality testing and psychic splitting through indicators such as distorted formal quality (FQ-) and the Thought and Perception Composite (TP-Comp). Likewise, the test is essential for differential diagnosis against autism or dissociative disorders, and demonstrates a solid empirical invulnerability against conscious malingering. The available evidence, however, warns about certain methodological challenges, especially the small size of some clinical samples and the existing tension between quantitative statistical rigor and the interpretive richness of projective assessment. It is concluded that the Rorschach transcends the simple mathematical sum of diagnostic markers, consolidating itself as an indispensable clinical tool to comprehensively understand the suffering and the fragmented internal world of the schizophrenic patient.
