Educación a enfermos ancianos pluripatológicos en la pauta y administración de su tratamiento

Abstract

El siguiente proyecto nace de la necesidad de medidas preventivas diseñadas para una población cada vez más longeva, con el objeto de mejorar su calidad de vida y reducir el riesgo de que padezcan episodios de gravedad que les puedan llevar a una situación de discapacidad permanente. La población diana estará formada por ancianos mayores de 80 años en condiciones de pluripatología y que consecuentemente consuman más de tres fármacos, pudiéndose denominar pacientes polimedicados. El proyecto se centra en este tipo de pacientes ya que el objetivo principal del mismo será brindar la suficiente formación e información de la manera más individualizada posible a sus participantes con el objeto de que sean ellos mismos los que detecten problemas en su tratamiento y así poder evitar errores evitables en la prescripción así como interacciones farmacológicas.

The next project is born out of the need of preventive measures designed for a population that is getting older, with the objective of improving their life’s quality and reducing the risk of suffering seriously episodes that could lead them to a disability permanent situation. The target population will be formed by elderly people aged 80 or over in polypathology conditions that consequently make them consume more than three drugs, making them able to be called polymedicated patients. The project is centered on this sort of patients given the fact that the main objective will be to provide them enough formation and information, in the most individualized way, so that they will be able to detect their own treatment problems and so avoid prescription evitable mistakes as well as drugs interactions.
