Manejo de dispositivos de vía aérea difícil: Proyecto educativo

Abstract

El manejo de la vía aérea difícil supone proteger y asegurar la vía aérea, con el fin de poder ventilar a cualquier tipo de paciente, con independencia de sus características morfológicas. En muchas situaciones donde esta no está prevista se crea una situación de urgencia constituyendo un problema que puede tener consecuencias importantes sobre el paciente como la muerte. Afortunadamente, la formación y la experiencia de profesionales de enfermería en este campo, ayudan a conseguir al anestesiólogo un control de la vía aérea así como la reducción de la morbimortalidad. Debido a que el manejo de los dispositivos cada vez es mayor, estos han evolucionado considerablemente desde la invención de la mascarilla laríngea, el uso tras la elaboración de guías y protocolos en el manejo de las distintas situaciones y el uso de dispositivos ópticos que mejoran la visión, de los que el fibrobroncoscopio sigue siendo el patrón oro a la hora del manejo en muchos casos de vía aérea difícil. De todo ello deriva la importancia del conocimiento, manejo y preparación de todos los dispositivos existentes a tales efectos, que la enfermería del área quirúrgica debe tener en este campo.

The management of the difficult airway suppose to protect and secure the airway in order to ventilate any patient, regardless their morphological characteristics. In many situations, where it is not provided, an emergency situation is created to be a problem that can have important consequences for the patient’s death. Fortunately, training and nurses’ experiences in this area help the anesthesiologist to get under control the airway and also, to reduce the morbidity and mortality. Because the management of devices is increasing and these have evolved considerably since the invention of the laryngeal mask, the use after the development of guidelines and protocols in handling/ management of different situations and the use of optical devices that enhance the vision, particularly the bronchoscope, which remains the gold standard when handling/ management in many cases of difficult airway. From all this, it comes the importance of knowledge, handling/ management and preparation off all devices for such purposes, as well as the nursing of the surgical area should be in this field.
