Resumen : | Antecedentes: El bruxismo es una actividad repetitiva basada en apretar y rechinar los
dientes y/o empujar la mandíbula. Hay muchos tratamientos para este trastorno, desde
farmacológico y odontológicos hasta fisioterápicos.
El tratamiento habitual fisioterápico consiste en abordar los puntos gatillo de los músculos
maseteros y temporales. A la hora de tratar los puntos gatillo, hay diferentes técnicas para
su inhibición.
Objetivos: Conocer la eficacia de la inclusión del abordaje de los puntos gatillo en los
músculos esternocleidomastoideos y suboccipitales en el tratamiento habitual de puntos
gatillo en los músculos masticatorios en pacientes bruxistas nocturnos con férula.
Metodología y métodos: Se ha diseñado un estudio analítico, experimental, prospectivo y
enmascarado a simple ciego. Formado por 58 sujetos de entre 30 y 40 años que acuden al
Hospital Doce de Octubre a la consulta de maxilofacial. Todos los sujetos están
diagnosticados de bruxismo nocturno y utilizan férula. La muestra se dividirá en dos
grupos de forma aleatoria: un grupo control y otro experimental. En el grupo control se
aplica el tratatamiento convencional que abarca los músculos maseteros y temporales, y el
grupo experimental se le incluye el abordaje de los puntos gatillo en los músculos
esternocleidomastoideos y suboccipitales. Se utiliza la técnica de Jones.
Se llevaa a cabo dos mediciones, una pre y otra post tratamiento. Se valora la apertura
bucal, el umbral de dolor a presión y la actividad muscular. Background: The bruxism is a repetitive activity based in clenching and chewing the teeth and/or pushing the jaw. There are many treatments for this patology: farmacologist, odontologist and physiotherapist. The conventional physiotherapist treatment consist in aproaching the trigger points of the masseter and temporal muscles. When treating the trigger points, there are differents technics that can be used. Objectives: To know the efficacy of the inclusión with the aproach of the trigger points in the sternocleidomastoid and suboccipital muscles in the conventional treatment of the trigger points in the masticatory muscles in nocturnal bruxism pacients with occlusal splints. Material and methods: An analitical, experimental, prospective and masked single-blind study has been designed. The sample is composed by 58 subjects who attend the maxilofacial consult in the Doce de Octubre Hospital. All the pacients have been diagnosed of nocturnal bruxism and they use a splint. The sample is divided in two aleatory groups: a control and an experimental group. In the control group the conventional treatment is applied which attends the masseter and temporal muscle and in the experimental group it is included the aproach of the trigger points in the sternocleidomastoid and suboccipital muscle. The Jones technic is used. Two measurements are conducted, one pre and another post treatment. It is valued the active mouth opening, pressure pain threshold and the muscular activity. |
