La llegada a un mundo en el que me miran con preocupación y desde lejos : establecimiento del vínculo maternofilial en la Unidad de Cuidados Intensivos Neonatales

Abstract

Los nacimientos prematuros son una emergencia de salud global. Si bien la tasa de supervivencia de los bebés prematuros ha mejorado, el adelanto del parto tiene múltiples consecuencias en el desarrollo biopsicosocial del bebé, teniendo en cuenta además que el ingreso en la Unidad de Cuidados Intensivos Neonatales (UCIN) es un factor clave en las consecuencias de la prematuridad. El objetivo principal del presente trabajo es explorar cómo la prematuridad en el contexto de la UCIN impacta en el establecimiento del vínculo maternofilial, así como la consecuente repercusión en el desarrollo del bebé.

El nacimiento prematuro junto al contexto de la UCIN tiene un impacto en las pautas de comportamiento del bebé, dificultando que la madre pueda interactuar con él. En este escenario, las representaciones maternas del vínculo con el bebé pueden verse distorsionadas, lo que puede llevar a la madre a sentir que su relación con su hijo es peligrosa. Consecuentemente, la madre puede activar operaciones defensivas, entre las que se incluyen las respuestas parentales insensibles durante la interacción con el bebé. De esta forma, los dos componentes del vínculo maternofilial (representaciones e interacciones) se ven afectados, siendo la principal variable mediadora que protege al vínculo de este peligro la seguridad de la historia de apego de la propia madre. Así mismo, se recogen una gran variedad de consecuencias que tiene la alteración del vínculo maternofilial en el desarrollo del niño. Sin embargo, finalmente se apunta hacia una investigación en la resiliencia del vínculo maternofilial para superar la adversidad, promoviendo una mirada ecológica y flexible sobre este.

Preterm birth is a global health emergency. Although the survival rate of preterm infants has improved, early delivery has multiple consequences on the biopsychosocial development of the newborn, especially considering that admission to the Neonatal Intensive Care Unit (NICU) is a key factor in the consequences of prematurity. The primary purpose of this paper is to examine the impact of prematurity in the NICU context on the establishment of mother-infant bonding and its subsequent effect on the baby's development.

Premature birth coupled with the NICU context has an impact on the infant's behavioral patterns, making it challenging for the mother to interact with her newborn. In this scenario, maternal representations of the bonding with her infant may become distorted, potentially causing the mother to perceive the relationship with her child as dangerous. Consequently, the mother may activate defensive operations, including insensitive parental responses during interaction with the infant. Consequently, both components of the mother-infant bonding (representations and interactions) may be compromised. The main mediating variable that can protect the bonding from this potential danger is the security of the mother's own attachment history. In addition, a great variety of consequences of the alteration of the mother-infant bonding in the child's development are reported. Nevertheless, it is finally suggested to investigate the resilience of the maternal-filial bonding in overcoming adversity, while promoting an ecological and flexible perspective of this bond.
