Abstract

Esta revisión sistemática analiza la relación entre las modalidades de lactancia y distintas dimensiones relacionales tempranas de la díada madre-bebé: el bonding o vínculo materno-infantil, la sensibilidad materna y la calidad de la interacción diádica. Para ello, se realizó una búsqueda en diferentes bases de datos (PubMed, PsycINFO, Psychology & Behavioral Sciences Collection y Cochrane). Se revisaron trabajos cuantitativos publicados entre 2015 y 2025, centrados en díadas madre-bebé durante los primeros 24 meses de vida, que compararan distintas modalidades de lactancia e incorporaran medidas relacionales tempranas. Tras el proceso de cribado, se incluyeron 19 estudios. Los resultados muestran cierta heterogeneidad. En el bonding materno-infantil, algunos estudios encontraron asociaciones positivas hacia la lactancia, sobre todo en comparación con lactancia artificial, mientras que otros no hallaron diferencias cuando se consideraban variables relacionadas con el bienestar y la experiencia subjetiva de la madre. En cambio, los dominios de sensibilidad materna y calidad de la interacción presentaron una dirección más consistente, con asociaciones positivas entre lactancia materna y responsividad, disponibilidad emocional, tacto afectivo o calidad del intercambio. La lactancia aparece, así, como un posible contexto relacional, de regulación y encuentro, y no como una garantía de vínculo. El cuidado clínico debería, entonces, centrarse menos en prescribir una forma de alimentar y más en sostener las condiciones emocionales, corporales y sociofamiliares en las que madre y bebé pueden encontrarse.

This systematic review examines the relationship between infant feeding modalities and several early relational dimensions of the mother-infant dyad: bonding, maternal sensitivity and the quality of dyadic interaction. A literature search was conducted across various scientific databases (PubMed, PsycINFO, Psychology & Behavioral Sciences Collection and Cochrane). Quantitative studies published between 2015 and 2025 were reviewed, focusing on mother-infant dyads during the first 24 months of life, comparing different feeding modalities and including measures of early relational processes. After the screening process, 19 studies were included. Findings show a certain degree of heterogeneity. Regarding mother-infant bonding, some studies found associations in favour of breastfeeding, especially when compared with formula feeding, whereas others found no significant differences once variables related to maternal wellbeing and the mother’s subjective feeding experience were considered. Conversely, the domains of maternal sensitivity and quality of interaction showed a more consistent direction, with positive associations between breastfeeding and responsiveness, emotional availability, affectionate touch or the quality of the dyadic exchange. Breastfeeding thus appears as a possible relational context, one of regulation and encounter, rather than as a guarantee of bonding. Therefore, clinical care should focus less on prescribing a particular way of feeding and more on supporting the emotional, bodily and sociofamilial conditions in which mother and baby can meet.
