Influencias del apego y psicopatología parental en adolescentes con TDAH
Abstract
Antecedentes: La seguridad en el apego favorece el desarrollo de las capacidades de
autorregulación, las cuales se encuentran muy afectadas en el TDAH. Los objetivos de
este estudio fueron: (i) analizar la asociación entre TDAH e inseguridad en el apego, y
(ii) estudiar la relación existente entre la psicopatología de los padres y la del
adolescente. Método: Se realizó un estudio transversal de casos y controles. La muestra
estaba compuesta por 30 adolescentes de entre 13 y 18 años (15 con TDAH y 15 sin
trastorno psiquiátrico), y por 20 padres de adolescentes con TDAH (11 madres y 9
padres). Se utilizaron los siguientes cuestionarios: CaMiR-R para la evaluación de las
representaciones de apego, SCL-90-R para la psicopatología paterna y CBCL para la de
los adolescentes. Resultados: respecto al primer objetivo, no se encontró asociación
entre la seguridad/inseguridad del apego y la presencia/ausencia de TDAH. Respecto al
segundo, se encontraron diferencias estadísticamente significativas en todas las escalas
del CBCL (salvo en las de somatizaciones, problemas sociales y la no aceptación de
normas) al comparar la psicopatología de los adolescentes con TDAH en los grupos de
padres (elevada psicopatología/baja psicopatología). Conclusiones: No se puede
afirmar con los datos de nuestra investigación que la inseguridad en el apego se
relacione con el diagnóstico TDAH. En cambio, la relación entre psicopatología paterna
y psicopatología del hijo es evidente y sugiere la necesidad de incluir intervenciones en
el subsistema parental y en la familia en el tratamiento de adolescentes TDAH. Background: attachment security favors the development of self-regulation skills,
which are very affected in ADHD. The objectives of this study were: (i) analyze the
association between ADHD and attachment insecurity, and (ii) study the existing relationship between parental and adolescents’ psychopathology. Method: A crosssectional
study of cases and controls was performed. The sample consisted of 30
adolescents between 13 and 18 years (15 with ADHD and 15 without psychiatric
disorder), and 20 parents of adolescents with ADHD (11 mothers and 9 fathers). The
following questionnaires were used: CAMIR-R for assessing attachment
representations, SCL-90-R for parental psychopathology and CBCL for adolescents’
psychopathology. Results: Regarding the first objective, no association between
security / insecurity of attachment and the presence / absence of ADHD was found.
Concerning the second objective, differences were found statistically in all scales of the
CBCL (except somatization, social problems and non-acceptance of standards) by
comparing the psychopathology of adolescents with ADHD in parent groups (high
psychopathology / low psychopathology). Conclusions: With the data from our
research we can´t affirm that attachment insecurity relates to the diagnosis of ADHD.
Instead, the relationship between parental psychopathology and child psychopathology
is evident and suggests the need to include interventions in the parental subsystem and
the family in the treatment of adolescents with ADHD.
