Relación entre las dificultades post-migratorias de personas desplazadas y el TEPT-C
Abstract
Los migrantes forzosos frecuentemente afrontan dificultades tras la migración que son constitutivas de un contexto de exclusión social. Asimismo, se trata de una población con una alta prevalencia de trastorno de estrés postraumático y trastorno de estrés postraumático complejo (TEPT-C). El trabajo pretende profundizar en la relación entre las condiciones post-migratorias de migrantes forzosos, su situación de exclusión social y el desarrollo de TEPT-C. Para ello, se revisa la bibliografía existente sobre el efecto de la exclusión social y las dificultades post-migratorias, encontrando relación entre estas y el TEPT-C. A continuación, se detalla el efecto de algunas dificultades específicas. Se abordan la inseguridad en el permiso de residencia, el acceso a la atención sanitaria y las condiciones socioeconómicas como factores políticos y económicos, y la soledad y falta de apoyo social, las barreras lingüísticas y la discriminación como factores personales y sociales. Se concluye que las dificultades post-migratorias se relacionan con un mayor riesgo de padecer TEPT-C. Existe más evidencia a favor del efecto de los factores personales y sociales sobre el desarrollo del TEPT-C que sobre factores políticos y económicos, lo cual tiene implicaciones terapéuticas. Forced migrants frequently face difficulties after migration which result in a social exclusion context. Additionally, refugees and asylum seekers suffer a high prevalence of post-traumatic stress disorder and complex post-traumatic stress disorder (CPTSD). The aim of this paper is to explore the relationship between post-migration factors, social exclusion and the development of CPTSD. To this aim, current literature on the effect of social exclusion and post-migration living difficulties is reviewed, finding evidence which connects those factors with CPTSD. Following, the effect of specific difficulties is detailed. The reviewed conditions are residential insecurity, access to health services and socioeconomic conditions, which are political and economic factors, as well as loneliness and lack of social support, language barriers and discrimination, which are personal and social factors. In conclusion, post migration living difficulties increase the risk of suffering CPTSD. There is greater evidence regarding the effect of personal and social factors on CPTSD compared to political and economic factors, which has treatment implications.
