Feedback del cliente como método para incrementar el progreso terapéutico y reducir el abandono prematuro: proceso y resultados tras 12 sesiones de terapia
Abstract
Recoger el feedback del cliente sobre el progreso y la alianza terapéutica de forma rutinaria ha
demostrado resultados positivos tanto en mejorar los resultados de la psicoterapia como en
disminuir el abandono prematuro. Dada la inexistencia de estudios realizados en población
española sobre el uso del feedback, el propósito del presente estudio tiene como objetivo analizar si
se producen estas mejoras tras implementar el Partners for Change Outcome Monitoring System
(PCOMS) como sistema de feedback en pacientes que asisten a la Unidad de Intervención
Psicosocial (UNINPSI) en Madrid (España). Se utilizó un diseño cuasiexperimental pretest-posttest
con un grupo control y sistema de doble cegado. Tras comparar el grupo experimental (n=13) con
el grupo control (n=7) pasadas 12 sesiones de tratamiento, no se ha podido demostrar que el
sistema de feedback reduce el número de abandonos ni que mejora el progreso terapéutico. Sin
embargo, este sistema de feedback ha permitido identificar qué pacientes se están deteriorando y/o
no han revelado ningún cambio y se han podido analizar las características que éstos presentan.
Adicionalmente, se ha encontrado una correlación positiva entre la alianza terapéutica en las
primeras sesiones de terapia y el resultado del tratamiento. Por último, se han discutido las
limitaciones del estudio, otros hallazgos e ideas para futuras investigaciones. Collecting client feedback routinely about the progress of the treatment and the therapeutic alliance
has demonstrated improved psychotherapy outcomes and reductions in premature dropout. Due to
the lack of feedback-use studies with a Spanish population, the purpose of this study was to analyze
if these improvements occur after implementing the Partners for Change Outcome Monitoring
System (PCOMS) as a feedback system in patients attending at the Psychosocial Intervention Unit
(UNINPSI) in Madrid (Spain). We used a quasi-experimental pretest-posttest design with a control
group and double blind system. After comparing the feedback condition (n=13) against the
treatment as usual (n=7) after 12 treatment sessions, it has not been possible to demonstrate that
neither the feedback system reduces the number of dropouts nor that it improves therapeutic
progress. However, this feedback system has allowed us to identify which patients are deteriorating
and/or have not revealed any changes and have been able to analyze the characteristics they
present. Furthermore, a positive correlation has been found between the therapeutic alliance in the
first sessions of therapy and the result of the treatment. Finally, limitations of the study, further
findings and ideas for future research are discussed.
