ESTUDIO EXPLORATORIO SOBRE INTERVENCIONES TERAPÉUTICAS NO INVASIVAS PARA EL TRATAMIENTO DE LA ENFERMEDAD DE WILLIS-EKBOM

Abstract

Este Trabajo Fin de Máster presenta un estudio clínico exploratorio sobre intervenciones terapéuticas no invasivas para la enfermedad de Willis-Ekbom (síndrome de piernas inquietas), un trastorno neurológico que afecta al 5-10% de los adultos y que carece de alternativas no farmacológicas suficientemente validadas. El núcleo del trabajo es un ensayo clínico domiciliario de dos semanas, doble ciego, con 65 participantes distribuidos en cuatro grupos paralelos (TENS y ejercicios de estiramiento isométrico, cada uno con su placebo), evaluados mediante las escalas IRLS, Epworth, EQ-5D-5L y RLSQoL. El TFM caracteriza en profundidad el perfil sociodemográfico y clínico de la muestra, analiza la relación entre hábitos de vida, comorbilidades y severidad inicial del síndrome, y presenta el análisis estadístico completo de los resultados. En resumen, el 81,5 % completó el estudio (53 participantes). La adherencia al registro diario fue ~50 %. Gracias al campo de texto libre se detectaron numerosas incidencias de seguridad no cuantificables con la escala numérica. Los ejercicios tratamiento redujeron el IRLS en 6,0 pts (p = 0,025) y el Epworth en 2,4 pts (p = 0,023); el TENS tratamiento redujo el IRLS en 4,1 pts (p = 0,032). Ningún grupo de tratamiento superó de forma estadísticamente relevante a su placebo. El documento concluye con una hoja de ruta hacia un futuro ensayo confirmatorio con mayor potencia estadística.
This Master's Thesis presents an exploratory clinical study on non-invasive therapeutic interventions for Willis-Ekbom disease (restless legs syndrome), a neurological disorder affecting 5-10% of adults for which sufficiently validated non-pharmacological alternatives are still lacking. The core of the work is a two-week, double-blind, home-based clinical trial with 65 participants distributed across four parallel groups (TENS and isometric stretching exercises, each with its own placebo), assessed using the IRLS, Epworth, EQ-5D-5L, and RLSQoL scales. The thesis thoroughly characterizes the sociodemographic and clinical profile of the sample, examines the relationship between lifestyle habits, comorbidities, and baseline symptom severity, and presents the full statistical analysis of the results. In summary, 81.5% completed the study (53 participants). Adherence to daily logging was ~50%. Thanks to the free-text field, numerous safety-related incidents were detected that could not be captured by the numeric scale. Exercise treatment reduced IRLS by 6.0 points (p = 0.025) and Epworth by 2.4 points (p = 0.023); TENS treatment reduced IRLS by 4.1 points (p = 0.032). Neither treatment group showed a statistically significant superiority over its placebo. The document concludes with a roadmap toward a future confirmatory trial with greater statistical power.
