dc.description.abstractLa Esclerosis Lateral Amiotrófica (ELA) es una enfermedad neurodegenerativa que afecta a las motoneuronas superiores e inferiores, provocando una pérdida gradual de la función motora. Los pacientes van perdiendo progresivamente la capacidad de moverse, hablar y tragar, y el fallecimiento suele producirse por insuficiencia respiratoria entre tres y cinco años tras el inicio de los síntomas. Uno de los principales problemas clínicos es el retraso en el diagnóstico, que supera en muchos casos los doce meses, debido a la falta de biomarcadores objetivos y no invasivos para la detección temprana de la enfermedad. Este trabajo investiga el uso de técnicas de Machine Learning aplicadas a biomarcadores obtenidos mediante ecografía muscular cuantitativa para clasificar pacientes con ELA frente a sujetos sanos. Se utilizó un dataset de 104 registros procedentes de 52 sujetos proporcionado por el Centro Universitario San Rafael de la Universidad Pontificia Comillas. Las variables analizadas incluyen biomarcadores ecográficos clásicos como el grosor muscular y la ecogenicidad, descriptores texturales obtenidos mediante la matriz de co-ocurrencia de niveles de gris (GLCM) y variables clínicas, evaluados en cuatro grupos musculares: bíceps braquial, flexores del antebrazo, cuádriceps femoral y tibial anterior. Se compararon siete algoritmos de clasificación mediante validación cruzada de diez particiones. El mejor modelo fue la Regresión Logística, alcanzando una accuracy del 90% y un área bajo la curva ROC de 0.973. El análisis específico del cuádriceps obtuvo un AUC de 0.977, y al incorporar las variables clínicas el rendimiento mejoró hasta una accuracy del 95% y un AUC de 0.989. Los descriptores texturales GLCM, especialmente la correlación del cuádriceps, resultaron ser los biomarcadores más relevantes. Estos resultados demuestran que la combinación de ecografía muscular e inteligencia artificial es una herramienta prometedora, accesible y no invasiva para apoyar el diagnóstico de la ELA.es-ES
dc.description.abstractAmyotrophic Lateral Sclerosis (ALS) is a progressive neurodegenerative disease affecting both upper and lower motor neurons, leading to a gradual loss of motor function. Patients progressively lose the ability to move, speak and swallow, with death typically occurring from respiratory failure within three to five years of symptom onset. One of the main clinical challenges is the diagnostic delay, which in many cases exceeds twelve months, due to the lack of objective and non-invasive biomarkers for early disease detection. This work proposes the use of Machine Learning techniques applied to biomarkers obtained from quantitative muscle ultrasound to classify ALS patients versus healthy controls. A dataset of 104 records from 52 subjects provided by the Centro Universitario San Rafael of Universidad Pontificia Comillas was used. The variables analysed include classical ultrasound biomarkers such as muscle thickness and echointensity, textural descriptors derived from the Gray-Level Co-occurrence Matrix (GLCM), and clinical variables, evaluated across four muscle groups: biceps brachii, forearm flexors, quadriceps femoris and tibialis anterior. Seven classification algorithms were compared using ten-fold cross-validation. The best-performing model was Logistic Regression, achieving an accuracy of 90% and an area under the ROC curve of 0.973. The specific analysis of the quadriceps yielded an AUC of 0.977, and incorporating clinical variables further improved performance to an accuracy of 95% and an AUC of 0.989. GLCM textural descriptors, particularly the quadriceps correlation parameter, were identified as the most relevant biomarkers. These results demonstrate that combining muscle ultrasound with artificial intelligence is a promising, accessible and non-invasive tool to support the clinical diagnosis of ALS.en-GB
