Psicodiagnóstico psicoanalítico con técnicas proyectivas

Abstract

This paper analyzes psychopathy through the Rorschach Test, coded according to the Comprehensive System by Exner (Exner, 2003) and integrated with information from the Psychopathy Checklist–Revised (PCL-R; Hare, 2003), with the aim of identifying the main psychopathic indicators in adult populations and exploring possible sex differences. Psychopathy is conceptualized as a complex construct that traditional diagnostic systems do not always adequately capture, as they focus on observable antisocial behavior and relegate the intrapsychic functioning of personality to a secondary level (Cleckley, 1988; Romero et al., 2011). To address this issue, a literature review was carried out using specialized databases, selecting studies published since the 1990s that jointly employ the Rorschach and the PCL-R in adult samples (Gacono, Meloy, & Heaven, 1990; Gacono, Meloy, & Bridges, 2000). The studies analyzed identify consistent patterns, such as low interpersonal interest, a scarcity of human and texture responses, difficulties in affect regulation, and egosyntonic aggression, whose assessment requires the use of specific indices such as the Extended Aggression Scores (Gacono, Bannatyne-Gacono, Meloy, & Baity, 2005) and the Traumatic Content Index (Armstrong & Loewenstein, 1990). Furthermore, several investigations point to differences between men and women with psychopathic traits in self-organization, affective experience, and relational style, suggesting distinct psychodynamic configurations as a function of sex (Rangel García, 1999; Fernández Belinchón, 2016; Cunliffe & Gacono, 2005, 2007; Smith, Gacono, & Cunliffe, 2018, 2020, 2021). Taken together, the findings support the usefulness of the Rorschach Test, in combination with the PCL-R, as a tool to deepen the understanding of the intrapsychic functioning of psychopathy and to improve its assessment, diagnosis, and intervention in clinical and forensic settings.
