Terapia de restricción del lado sano frente al uso de videojuegos, tras accidente cerebrovascular

Abstract

Fundamento y objetivo: Debido al aumento del número de supervivientes tras accidente cerebrovascular y a la escasa atención a las secuelas en el miembro superior, las nuevas técnicas desarrolladas cobran gran importancia. El objetivo es comparar la efectividad de la terapia por restricción del lado sano, frente al uso de videojuegos, en el aumento de la funcionalidad del miembro superior. Método: Ensayo clínico aleatorizado con simple ciego modificado, cegando al evaluador. Las variables son la funcionalidad, medida con la escala de Fugl-Meyer; la calidad de vida, medida con la ECVI-38; el dolor, medido con algómetro; la amplitud articular y la fuerza máxima isométrica, medidas con el dinamómetro isocinético. Se requieren 98 sujetos adultos hemiparésicos crónicos tras accidente cerebrovascular isquémico, con omalgia y limitación en la amplitud articular y la fuerza máxima isométrica. Por lo tanto, con alteraciones en la funcionalidad y calidad de vida. A un grupo se le aplica la terapia por restricción del lado sano, junto a electroestimulación, y en el otro grupo se utilizan los videojuegos, junto a electroestimulación. La hipótesis es que la terapia por restricción del lado sano es más efectiva que el uso de videojuegos, en cuanto al aumento de la funcionalidad del miembro superior. Se medirán los resultados antes y después de la aplicación de las intervenciones. La prueba estadística prevista es la T de Student para muestras independientes
Background and purpose: Due to the stroke survivals increased and the limited assistance to the upper limb impairments, newly developed techniques gain more importance. The aim of this study is to compare the constraint-induced movement therapy effectiveness, against the use of video games, in enhancing the upper limb functionality. Methods: Randomized clinical trial with modified single-blind experiment. The variables are the functionality, measured with Fugl-Meyer Assesment of Motor Recovery after stroke; the quality of life, measured with the ECVI-38; pain, measured with algometer; range of motion and maximun isometric strength (torque), measured with an isokinetic dynamometer. It requires 98 adult subjects with chronic hemiparesis after ischemic stroke. They should have shoulder pain, limited range of motion and maximal isometric strength. So, they suffer functional and quality of life affectations. The constraint-induced movement therapy next to electrostimulation is given to one group and to the other group videogames after electrostimulation will be applied. The hypothesis is that constraint-induced movement therapy is more effective than the use of video games, in terms of increasing upper limb functionality. The results will be measured before and after the application of these techniques. Independent T-test is the expected test for the statistical analysis.
