Impacto de la afasia postictus en la comunicación funcional, calidad de vida, percepción de salud y depresión. Estudio caso-control
Abstract
Objetivos: Evaluar el impacto de la afasia postictus en la comunicación funcional, percepción de calidad de vida, de salud y síntomas depresivos.
Material y Métodos: Estudio caso-control. Casos: pacientes con afasia post-ictus incluidos en el ensayo clínico DULCINEA (NCT04289493). Controles: personas sanas reclutadas mediante un muestreo no probabilístico de bola de nieve. Todos los sujetos completaron cuestionarios que medían las variables comunicación (Comunicative Activity Log -CAL, subescala del Stroke Aphasia Quality of Life Scale-SAQOL-39), percepción de salud (General Health Questionnaire-GHQ-12) y de calidad de vida (SAQOL-39) y depresión en pacientes con afasia (Stroke Aphasic Depression Questionnaire-SADQ10). Se llevó a cabo la Prueba de Shapiro-Wilkison y el Test de Levene (valorar normalidad) y la U de Mann-Whitney (diferencias entre muestras) con la d de Cohen (tamaño del efecto).
Resultados: Se incluyeron 11 pacientes con afasia post-ictus [3 mujeres, edad media: 62 años, desviación típica (DT): 13,28] y 73 sujetos sanos (42 mujeres, edad media: 53,67 años, DT 6,67). Los pacientes con afasia post-ictus obtuvieron menos puntuaciones que la población sana en las variables percepción de calidad de vida (SAQOL-39: U=124, p>,001, d=0,61 ) y percepción de salud (GHQ-12: U=124, p=,004, d=0,36 ). Sin embargo, aunque las puntuaciones totales en las variables comunicación funcional fueron significativamente menores en la población con afasia (CAL: U= 158, p=.001, d=0.61; subescala comunicación SAQOL-39 (U=26, p>,001, d=0,61 ) y subescala frecuencia (casos media=54,6, DT=8; controles: media=64,7 y DT=8,93; U=158, p>,001, d=0,61), no observamos diferencias en la calidad percibida de la comunicación (CAL, subescala calidad: afasia: media=47, DT=13,9 y sana: media=41,9 y DT=6,98; U=300, p=0,178) y los pacientes con afasia refirieron menos síntomas depresivos que la población sana (SADQ-10: (casos: media=10.5, DT=3,72; controles: media=14,5, DT=3,86; U=182, p=,004, d=0,55).
Conclusiones: Aunque las dificultades de comunicación impactan en la calidad de vida de los pacientes con afasia post-ictus, su percepción de calidad de comunicación es similar a la población sana y refieren menos síntomas depresivos. Material and Methods: Case-control study. Cases: patients with post-stroke aphasia included in the DULCINEA clinical trial (NCT04289493). Controls: healthy people recruited using non-probabilistic snowball sampling. All the subjects completed questionnaires that measured the communication variables (Comunicative Activity Log -CAL, subscale of the Stroke Aphasia Quality of Life Scale-SAQOL-39), health perception (General Health Questionnaire-GHQ-12) and quality of life (SAQOL -39) and depression in patients with aphasia (Stroke Aphasic Depression Questionnaire-SADQ10). The Shapiro-Wilkison test and Levene's test (assess normality) and the Mann-Whitney U (differences between samples) with Cohen's d (effect size) were carried out.
Results: 11 patients with post-stroke aphasia [3 women, mean age: 62 years, standard deviation (SD): 13.28] and 73 healthy subjects (42 women, mean age: 53.67 years, SD 6, 67). Patients with post-stroke aphasia obtained lower scores than the healthy population in the variables perception of quality of life (SAQOL-39: U=124, p>.001, d=0.61) and perception of health (GHQ-12 : U=124, p=.004, d=0.36). However, although the total scores in the functional communication variables were significantly lower in the population with aphasia (CAL: U= 158, p=.001, d=0.61; SAQOL-39 communication subscale (U=26, p>.001 , d=0.61) and frequency subscale (mean cases=54.6, SD=8; controls: mean=64.7 and SD=8.93; U=158, p>.001, d=0.61 ), we did not observe differences in the perceived quality of communication (CAL, quality subscale: aphasia: mean=47, SD=13.9 and healthy: mean=41.9 and SD=6.98; U=300, p= 0.178) and patients with aphasia reported fewer depressive symptoms than the healthy population (SADQ-10: (cases: mean=10.5, SD=3.72; controls: mean=14.5, SD=3.86; U=182 , p=.004, d=0.55).
Conclusions: Although communication difficulties impact the quality of life of patients with post-stroke aphasia, their perception of communication quality is similar to that of the healthy population and they report fewer depressive symptoms.
